Required Registration Documents

The following documents must be uploaded to our registration system, SchoolMint, for each student.

  1. Scholar’s Birth Certificate (no update needed of it’s already in the form).
  2. Primary Contact’s Valid ID (no update needed if it’s already in the form and still valid).
  3. Report card (new students only, not for returning students).
  4. Two (2) Up-to-date Proof of Address Documents from last 60 days (three (3) forms of proof required for Yonkers residents). See acceptable documents, below.
  5. IEP or 504 Plan (if applicable).

ACCEPTABLE FORMS OF PROOF-OF-ADDRESS

Amani’s neighboring school districts (Mount Vernon, Yonkers, NYC, New Rochelle and others) require updated proof of address documents for every child attending a charter school, each year.

(2 documents required for Mt. Vernon residents, 3 for Yonkers, 2 for everywhere else)

*Your lease, deed or mortgage statement for where you reside.

*A utility bill (ConEd), in your name, dated in the last 60 days. 

*A water bill for your residence, dated within the last 90 days.

*A bill for cable TV or Internet service (Spectrum, Optimum, FIOS) that includes your name and your address, dated in the last 60 days.

*A document on letterhead from a federal, state or local government agency that includes your name and address and is also dated within the last 60 days.  Government agency documents may come from agencies such as, but not limited to:

//The Internal Revenue Service

//The New York State Office of Children and Family Services (OCSF)

//Westchester County Agencies or local city government agencies

*A current property tax bill for your residence.

*A rent receipt that includes your address of residence, dated within the past 60 days. (Not handwritten, please.)

*An income tax form for the last calendar year.

*Official payroll documentation from your employer, dated within the past 60 days.  This can be a paystub or a payroll receipt with your name and address on it.

*Voter registration documents, such as a recent voter reminder postcard from the Board of Elections, which includes your name and address of residence.

*Unexpired membership documents based upon residency (like a neighborhood residents’ association), which incudes your name and address of residence.

*Evidence of custody of your child, including custody orders or guardianship papers.  These documents must have been issued within the past 60 days and must include the name of your students as well as your home address.

Certain documents will NOT be accepted as proof of residency or proof of address.  These documents are:

-A letter of employment

-A mobile phone bill

-A credit card

-Medical insurance cards

Health Records

Each student’s most recent medical exam form and immunization record is required.

Health records are required for all students in all grades, each year.
